What the patient opens
A patient signs in and keeps an account and a profile. The portal shows medical records, historical health information, and clinical documents in a form a patient can read. When something is missing or unavailable, the portal says so instead of presenting a broken view.
What stays in the hospital
Clinical and operational systems stay where they are. A connection layer and APIs ask those sources and present one view. The screen is not tied to a single source system, and the patient does not have to know which system held the record.
Who is allowed to see it
Access is authenticated and limited by role. Sessions are controlled. Actions are written to an audit log. The portal is monitored as a production system that patients use.
